Families who have children and young people with special needs very often deal with similar life styles. We have all had concerns ranging from education,acess to services,respite and other issues directly related to their childrens needs. Parents of disabled children bring families together for friendship, to share information and to support one another.

The forum was created by parents who have children with special needs and is run by parents so we are all in this together. With us you will realise you are not alone

Parents of disabled children launched in October 2009 and we are growing day by day and are fastly becoming a major support resource for all

where do i ask i want to see daughters hospital records?

am not happy with some things so i want to see chloes hospital records how where do i ask for them.?

You can request informal access to records by speaking to the professional staff involved in your care.

If you think that some of the information in your records is inaccurate you are entitled by law to ask for it to be corrected.

In special circumstances the law allows the PCT not to show you your information, such as if they consider it harmful to you, or another person’s physical and/or mental wellbeing.

For formal access to your records you must make a request in writing or by e-mail to the relevant contact below. Where you are under the care of more than one of the partner organisations, this person will liaise with the other contacts to source the information.

Please note that under the provisions of the Data Protection Act 1998 you may be charged to access your records (up to a maximum of £10 per request), with an overall limit of £50 where copies are provided.

The people to contact are as follows:
In your case i would suggest the first one as its the PCT and will divulge all details held.
But then the hospital one would be more specific so maybe the 2nd is the best option?
