DLA Form reports

Do I need to get reports from all of Findlay's consultants and therapists to send off with the form or do I just send off the form and the DLA people will contact the consultants themselves? If I need to send the reports would I be able to send the reports after the form if I won't get the reports in enough time to meet the deadline for getting the form back in? Sorry if this is a stupid question It's my first time filling in a DLA form and I'm a bit confused (and rather stressed as it's not the easiest form when you are trying to explain quite a few different things as I am sure many of you are aware!) |

I've never sent any forms in for Zaks claim. He is pretty self explanitory, and they will contact his consultant/nurse/physio etc. if they have any queries.

There is no obligation to provide any reports however the stronger you build your case the easier the process will be so if you have reports already do send them as it will speed it up

Thank you both. His S&L therapist, dietician and I think his gastro consultant as well have already written/ are writing reports for me so I will attach those to the form when they arrive through.
At least those 3 reports will cover communication issues, oral aversion, gastrostomy and tube feeding and his stomach and bowel probs and then the DLA people can contact his other consultants, physio etc if they want 'proof' about his other issues. |
